Take a look at this tool called SpotNote3.1.

SpotNote 3.1 is a commenting tool that allows the user to post
electronic notes directly to AutoCAD drawings they are editing. In
addition, these notes stay with the drawings as it is sent to various
    recipients. Unlike many
other "sticky-note" software which are used for providing personal
reminders, SpotNotes are used for professional communciations.

Cut out secondary documentation and emails to describe what you
    changed in a
document for a team member or co-worker. Why should you have to go
    back and
forth between applications to figure out what comment goes with what
    section
of the document. Instead, use SpotNote to simply attach a note in the
    exact
place you want it to be seen. Communicate with the confidence that
    what you
have to say, will be in the place you want to say it.

Visit www.spotnote.com to download a free 30 day trial.
