Why is it that sometimes a rectangle can be drawn by dragging the box to the
approximate size and other times I have to pick two corners with a line?

there are 2 rectangle buttons. one for vert and horz lines. the other
is a rectangle that can be rotated

 

Sometimes you are using the rectangle tool and other times you are
using the polygon tool. The rectangle puts horizontal and vertical
relationships on the sketch entities and the polygon parallel and
perpendicular relationships on the sketch entities.
