PDF Distiller Custom Page Sizes

Once upon a time I was able to create at least one custom page size in Adobe Distiller. I would create a PC3 and a PMP and would have the option of printing to the standard sizes and one large size (A1). Now, with autocad 2005 the very same PC3 with the very same PMP show only the standard small page sizes (11x17, ltr, legal, etc.). My custom size is lost. Even if I try to create a new PC3 from the distiller driver I see no options for custom page sizes at all. Why is this? Is there a better way to print to PDF? An autocad PDF driver perhaps?
I need to create PDF's of A1 and A0 size sheets and though I have been doing this for years I am now completely lost! Please help.

    Dean,

Do you have any specific suggestions as far as PDF generation?

Acrobat 6 has been nothing but a headache.

Acrobat 5 required extensive tweaking to make it do large custom page sizes
    (A1, E, etc)...

Lately, i've been trying CUTEPDF, but there's no way to get it to batch plot
PDF files (something that i do regularly)
