Has anyone addressed how to get leaders to align "correctly" with multiline
leader text? Like the leader at the left side of the note should align
vertically at the middle of the top line of the note and a leader at the
right side should align vertically at the middle of the bottom line of the
    note.

Yes but how can these settings be modified after the leader has been drawn?
For instance, if you draw a leader with the text to the right and the leader
to the left, and then move the text way left such that AutoCAD reverses the
objects (text left, leader right) it moves the leader to the vertical middle
    of the multiline text.

You could try selecting the offending leaders' text objects, and changing
their justification using the properties palette. Clumsy, but it will work.

Sorry, that does not work. It places the leader in the wrong place.
