cartouche with automated attributes

I would like to make a cartouche were the file name and date attributes are
automatically taken from the dwg file.

Some one can help me ?

Here's something we've been using and tweaking since 1998 -- feel free to
select the fragments you need to achieve your goal; note that this is not a
complete program and will not work by itself (it calls other functions not
included here) but it does contain the parts you need to extract file name and
date attributes, among other things.
